Liability Waiver - I understand that any use of facilities, equipment, parks and/or participation in and/or observation of Liberty Parks and Recreation programs is voluntary and may involve risk of bodily injury or property damage and I agree to use such facilities and equipment in an as-is condition and assume any and all such risks both known and unknown. I understand that it is up to me to consult with physicians and other professionals to make sure that I can safely participate in activities and events with Liberty Parks and Recreation.
Parent/Guardian Release - I, on behalf of myself, my spouse, heirs, assigns, and the participants named herein, assume all risks and hazards related to the participants’ participation in any activity conducted with Liberty Parks and Recreation. Further, I do hereby release, waive, hold harmless, and forever discharge the City of Liberty, its elected and appointed officials, employees, liability insurer, contractors, and all those who are or that may be liable on its or their behalf (“Released Parties”) from any and all liability, claims, damages, demands, judgments, losses, penalties, costs, expenses, or any sum of money (including expert and attorney fees), or causes of action whatever kind, either in law or equity, whether due to negligence or otherwise, whether caused by me or others which arise or may hereafter arise out of or in any way connected with the participants’ participation with Liberty Parks and Recreation to the fullest extent permitted by law. I hereby assume all risk of injury, damage, losses, and liability and covenant not to sue and waive any right of recovery from or to bring suit against the Released Parties, to the fullest extent permitted by law, for any illness, personal injury, death, or other consequence arising out of my own, my spouse, heirs, assigns, and the participants named herein voluntary participation or observation in these activities.
Photo/Video Release - The Liberty Parks and Recreation Department frequently takes photos or video of members enjoying programs, special events, parks and facilities. This content is used for the City of Liberty publications, websites, social media, etc. They are used at the discretion of the City of Liberty. By signing this release, I grant the Liberty Parks and Recreation Department permission to use such content as provided herein and understand such content is the sole property of the City of Liberty.
Outside groups requesting access to members and participants of the Parks and Recreation Department facilities and programs for photos and videos must first obtain permission from all participants. Photos of individuals under the age of 18 must first obtain permission from parents/guardians. Photos and videos are strictly prohibited in private areas of all facilities (restrooms, dressing rooms, etc.).
I agree that this waiver and release shall be binding upon me, my heirs, executors, administrators, guardians and assigns.
