Filling out the form takes less than 5 minutes and will allow CCPHC to send you updates regarding your eligibility and upcoming opportunities to get vaccinated.

As you may have seen in the news, every state has different "priority tiers" for administering the vaccination and will move through the tiers at varying paces based on vaccine availability. You can find Missouri's tiers online here » 

CCPHC is only one of the providers offering vaccinations. This is NOT a registration form for vaccination - only an interest form. Continue to be on the lookout for opportunities to be vaccinated through your doctor, another health care provider, a mass vaccination clinic, a retail pharmacy like CVS or Walgreens or through your employer.

While the State of Missouri has activated Phase 1A, Phase 1B-Tier 1 and Phase 1B - Tier 2, it is important to note that due to limited vaccine supply, the Clay County Public Health Center is only vaccinating those in Phase 1A (health care workers) at this time.
